Welcome to Canyons Ski & Snowboard School liability wavier form instruction page. To make your first lessons even easier, please complete and submit a liability form now using one of the following options.
Online Form
By using this process, we automatically update your Canyons Ski & Snowboard School Customer File. This allows you to quickly complete this process and doesn’t require any faxing or e-mail.
You will be using our online storefront to “purchase” a liability form (don’t worry, it costs nothing!). Remember, you will need to “purchase” a form for each lesson participant, so you will have to look up each family member. For those under 18, you will have to fill out a parent / guardian section.
While this is easy to do, it requires a bit of clicking and navigation. Just follow these simple instructions:
- To start, click the following link: Online Liability Form Link
- Next, click the "Add to Cart" button
- Follow the instructions to find the person taking the lesson (you or your child) and when you have the correct guest, click continue
- Read the form, and at the bottom of the page, Click the “agree” check box, to activate the continue button, click continue.
This is the process for one liability form, if you need to add another participant, click “continue shopping” and select the Liability Form from the list to process another guest.
Once everyone in the family has a Liability Form accepted, click the “Check Out” button and you are done.
Hard Copy Form
If you would like to just fill-out a hard copy of the form and e-mail or fax it to us, just click the link below this section to download the form and send it to us. Remember, you can put your entire family on one form and be finished, but requires VERY LEGIBLE handwriting.
