Liability Processing

Welcome to Canyons Ski and Snowboard School liability form page. To make your first morning of lessons easier, please complete a liability form now using one of the three options below. Since some technologies act up on different computers, we have created three methods for completing the form: high tech, “mid”-tech, and low tech. Please select one of the options below to process your liability forms for the 2011-2012 season.

High-Tech: Web-based Agreement Processing 

This process updates your Canyons Ski and Snowboard School Customer File automatically, which means that as soon as you complete this method, there is no question that we have your liability on file – no faxing, no e-mailing. 

At a basic level, you will be using the online sales outlet to “purchase” a liability form (don’t worry, it costs nothing!), looking up your customer file and accepting terms of the liability form. The trick here is that you will need to “purchase” a form for each lesson participant, so you will have to look up each family member. For those under 18, you will have to fill out a parent/guardian section. 

While this is easy to do, it requires a bit of clicking and web navigation. It’s not very complicated, but we did spell out specific directions below to help you through:

Click the following link - Online Liability Form Link and then click the "Add to Cart" button next.

Follow the instructions to find the person taking the lesson (you or your child)

When you have the correct guest, click continue

Read the form, and at the bottom of the page, Click the “agree” check box, to activate the continue button, click continue.

This is the process for one liability.

If you need to add another participant, click “continue shopping” and select the Liability Form from the list to process another guest or

Once everyone in the family has a Liability Form accepted, click the “Check Out” button and you are done.

“Mid-Tech”: Electronic PDF Form Processing

This process uses an Acrobat Fillable PDF form which you can submit to us via e-mail. While the PDF works on most computers, some computers don’t like the Fillable PDF. 

To open the file, click the link below. Please read and fill out the form. 

Please put the Head of Household (mom/dad/guardian) in the first participant line. Family members with the same last name can go on one form. 

You will need to build a “digital signature file”, if you don’t already have one, that you store on your computer. The signature process is easy and instructions are automatically provided when you get to that section of the form. 

You will also be prompted to save a copy of the file after you add the electronic signature – this is normal behavior for the program. 

When you have everyone on the form, and all of the signatures in place, you can send the form by clicking the “Submit” button (found on the right of the toolbar). We will receive the form as an e-mail, and will then update your customer file.

Low-Tech: Faxed Paper Form Processing

If you wish to fill out a paper form by hand, click the link below this section for a PDF version of the form which you must print, fill out by hand and fax (or scan and email) to us using the contact information below. You can put your entire family on one form and be finished, but requires VERY LEGIBLE handwriting.
